Office Guidelines

Urban Health is deeply committed to serving our patients with the utmost dedication and care.  In order to fulfill this objective we ask your cooperation with the following guidelines:

Appointments

We appreciate our patients keeping their follow-up appointments after they have had lab work, diagnostics tests and primary care visits.  Test results CANNOT be given over the phone; therefore it is extremely important that you keep your follow-up appointment to discuss your test results and to determine the best course of action based on these.  We kindly ask that you provide us with a 24-hour notice should you need to cancel or reschedule your appointment.

Prescriptions

As your primary care providers it is extremely important that you bring ALL your medications to each office visit to allow us to keep an up-to-date list of all your prescription medications (including those prescribed by other providers).  Please remember to ask the doctor at the time of your appointment if you need to refill your prescriptions.  Our staff certainly understands that we may forget from time to time; however if you do not request a needed refill at the time of your appointment it may result in a delay or serious time lapse occurring before your needed medications can be obtained.  In the event you do forget you may call and leave a refill request message with our nursing staff.  Please allow 48 hours for your prescriptions to be filled.  Remember that controlled substance medications CANNOT be refilled over the phone.  You will need to make an appointment with one of our providers.

Be aware that Urban Health does not accept faxes from pharmacies.

Referrals

We strive to expedite your referrals and authorization requests; however, most requests require us to contact your insurance company.  Therefore, it may take up to 3 business days to process all referrals and authorizations.

Payments

Payment in full for deductibles, copays or any outstanding balance, is expected at the time of service.  Regrettably, you may be asked to reschedule your appointment if you are unable to comply with this policy, except in emergency situations.  Upon financial hardship, we ask that you kindly notify our office immediately to work on a payment plan.
